Required Documents

  I-751 Removal of Conditional Status The following is a list of documents to assist in the removal of conditional status.  Do not worry if you do not have every document.  Our office can assist you in the preparation of wills, power of attorney, and power of attorney for health care.

  • A mortgage or rental agreement signed by Husband and Wife
  • The front page of IRS Form 1040  showing filing status of married filing joint return.
  • IRS Form 1040 for the previous years again showing married filing joint return.
  • Copies of bank statements showing both Husband and Wife
  • Investments  showing both Husband and Wife as the owners.
  • Photocopy of a Certificate of Title showing both Husband and Wife
  • Insurance forms showing both Husband and Wife as the insured.
  • Designation the spouse as the beneficiary of his profit sharing plan.
  • Photo copies of the Last Will and Testament  showing the spouse as beneficiary.
  • Living Will showing the spouse as the authority under the Living Will.
  • Durable power of Attorney for Health Care giving the spouse authority under the Durable Power of Attorney.
  • Utilities bill with both spouses names
  • Photographs of both spouses
  • 2 pictures passport style of each
  • Affidavits of persons who know both spouses
  • Evidence of Husband's registration with Selective Service, if Husband is  18 through 25 and Husband is not a United States Citizen.
  • Birth Certificate of child(ren) born in the marriage

Criminal Issues
